  • How do I submit a prescription?

    Prescriptions and enrollment forms can be mailed to:

    Please be sure your date of birth is included on your prescription. If you do not have a hard copy prescription to mail, you can ask your physician to submit one to Eagle Pharmacy via:

    In accordance with the Board of Pharmacy Regulations, only a physician may submit a prescription through fax, phone, or ePrescribe. Patients may mail original, hard copy prescriptions to Eagle Pharmacy. Eagle Pharmacy will fill medications upon receipt unless otherwise noted on the prescription to hold/profile by the patient or physician.

  • What happens once I send in the enrollment form and my prescription?
    How do I check the status of my order?

    Once we receive your completed enrollment form and prescription information, we will process your order, and your medication will be shipped directly to your home within
    5 to 9 days, as long as no order processing issues arise. To check the status of your order, please visit and sign in on the secure Eagle Pharmacy portal.

  • How do I order a prescription refill?

    You can refill your prescriptions by visiting and signing in on the secure Eagle Pharmacy portal or by calling 1-855-892-3438. Patient Support Service representatives are available Monday through Friday, 8:00 am–10:00 pm EST, excluding holidays. In emergency situations only, a pharmacist is available after hours to assist with previously dispensed prescriptions.

    You can also elect to refill your medication through the automated refill program. Each month, your medication will automatically be shipped directly to your home; the credit card on file will be charged prior to shipping. Patients are responsible for all shipments and co-pays while enrolled in this optional service. You may opt in or out at any time by speaking with one of our Patient Support Service representatives at 1-855-892-3438.

Aggrenox® (aspirin/extended-release dipyridamole) 25 mg/200 mg capsules is a prescription medicine used to lower the risk of stroke in people who have had a “mini-stroke” (transient ischemic attack or TIA) or stroke due to a blood clot.

AGGRENOX should be avoided in patients who are allergic to any ingredient in AGGRENOX, or allergic to non-steroidal anti-inflammatory drugs (NSAIDs), or who have the combination of asthma, runny nose, and nasal polyps. AGGRENOX should not be given to a child or teenager.

AGGRENOX increases the risk of bleeding, including bleeding into the brain, stomach or intestines. Any bleeding you have may take longer to stop when you are taking AGGRENOX.

AGGRENOX should be avoided by patients with a history of stomach ulcers or those who drink three or more alcoholic drinks a day, as these can increase the risk of bleeding. Patients should tell their doctor about all medicines they are taking, especially blood thinners, heparin, warfarin, NSAIDs, heart medicines, or medicines for high blood pressure, including diuretics (“water pills”).

AGGRENOX should be avoided during pregnancy, especially in the third trimester. AGGRENOX should be avoided in patients with severe liver or kidney problems. The most common side effects of AGGRENOX are headache, upset stomach, and diarrhea.

By enrolling, I elect to receive the branded product and acknowledge that no generic substitution will be offered (if applicable). Should I wish to receive a generic product in the future, I will call 1-855-892-3438 to opt out of this program.

Patients without insurance restrictions and co-pays of $265 or less will pay a $10 co-pay for a one-month supply of AGGRENOX. Patients whose co-pay is less than $10 will continue to pay their usual co-pay. For patients whose co-pay is between $265 and $290, they will pay that amount minus the maximum savings benefit of $255 per month. No one will pay more than $35. Patients with insurance restrictions or co-pays of more than $290 will not be processed through insurance and will pay $35 for a one-month supply (60 capsules) of AGGRENOX.

If you have Medicare Part D, Medicaid or a similar state or federally funded medical assistance program, you will pay a cash price of $35 for a one-month supply of AGGRENOX. All Medicare Part D orders are processed without the use of insurance and cannot be applied to Part D true out of pocket (TrOOP) costs. Taxes may apply.
